Maintenance Plan

For patients maintaining oral health with minimal treatment needs.
Preventive Health Services
- 1 comprehensive or periodic exam per year
- 2 cleanings per year (excludes perio maintenance)
- Routine diagnostic x-rays
Member Savings
- 20% off basic treatment (fillings, root canals, extractions, periodontal services, IV sedation)
- 15% off major treatment (crowns, bridges, dentures, partials, ortho)
Recovery Plan

For patients rebuilding oral health with extensive treatment needs.

this is a discount plan, not a dental insurance plan. exclusions apply:
- It cannot be used in conjunction with another dental plan, dental insurance, or financing program such as CareCredit.
- This plan is only honored at Woodside Dental Group. It cannot be used at any other dental or specialist office.
- This plan cannot be used for hospitalization or hospital charges of any kind.

- Any unused services within the patient membership are nontransferable to other plan members regardless of the service.
- Any missed appointments or cancellations not made within 48 hours of the appointment may be charged a cancellation fee.
- All payments are due in full at the time services are rendered.
- No refunds.

- Membership fees are broken up into two payments: one at the time of registration and one the following month.
