Health Creator Plan

Patients with planned care, moderate amount of existing dental work to maintain, and a history of risk factors.

this is a discount plan, not a dental insurance plan. exclusions apply:
- It cannot be used in conjunction with another dental plan, dental insurance, or financing program such as CareCredit.
- This plan is only honored at Woodside Dental Group. It cannot be used at any other dental or specialist office.
- This plan cannot be used for hospitalization or hospital charges of any kind.

- Any unused services within the patient membership are nontransferable to other plan members regardless of the service.
- Any missed appointments or cancellations not made within 48 hours of the appointment may be charged a cancellation fee.
- All payments are due in full at the time services are rendered.
- No refunds.

- Membership fees are broken up into two payments: one at the time of registration and one the following month.
